The OCI-4 is a brief, four-question screening tool used to identify symptoms commonly associated with obsessive-compulsive disorder (OCD). It asks about everyday experiences such as intrusive thoughts, repeated checking, cleaning or washing behaviors, and the need for things to feel “just right.” By rating how much these experiences have bothered you over the past month, you get a clearer sense of how they may be affecting your daily life.

The OCI-4 is used to screen for OCD and better understand how distressing these symptoms feel for you. This short questionnaire can help you notice patterns over time and decide whether further evaluation or support might be helpful.
