These evidence-based screening tools are designed to support diagnostic clarity and care planning. Each completed screener links directly to the patient’s chart, allowing you to review responses, track progress over time, and tailor treatment to the patient’s needs. Screeners can be administered during appointments or sent to patients for self-guided completion in advance.

Depression

  • Self-guided: PHQ-9(Patient Health Questionnaire-9)

  • Provider-administered: MADRS (Montgomery–Åsberg Depression Rating Scale)

Generalized Anxiety Disorder

  • Self-guided: GAD-7(Generalized Anxiety Disorder 7-item)

PTSD / Trauma

  • Self-guided: PC-PTSD-5(Primary Care PTSD Screen) for general screen or PCL5 (PTSD Checklist for DSM-5) for after a traumatic event

  • Provider-administered: CAPS-5 (Clinician-Administered PTSD Scale for DSM-5)

Bipolar Disorder
